1. What information does LifeOnKey store?
  a. Personal and Medical data:
    i. From LifeOnKey members.
    ii. From others: hospitals, clinics, physicians etc. (subject to your authorisation)
    All medical and personal information are protected both physically and technologically. The LifeOnKey system is secured with SSL technology, compatible with HIPAA standards.
  b. General anonymous information from users of the site.
  c. Other information (advertisers, links, etc.)
 
2. How does LifeOnKey store and retrieve information?
  Personal and Medical Data:Your information may only be retrieved using your username, ID# and personal password, or by those you have specifically authorized to obtain your personal and medical information. LifeOnKey may not access your information without your password and may not bypass this password unless required by law. However, you may authorise LifeOnKey to divulge your information in cases of emergency.
 
3. What information does LifeOnKey provide:
  a. To you as a user: access to all your medical data stored on your personal virtual file.
  b.  To those the user authorised:
    Your information may only be retrieved using your username ID# and personal password. Therefore access is available only to those you have given your username, ID# and password to, or to physicians you have specifically authorized. If you wish that your medical information be accessible to any healthcare provider (be it a hospital or a doctor), you must specify this specifically.
  c. External relationships:
    i. Medical institutions: LifeOnKey provides aggregate statistical information for the use of medical institutions. This information is anonymous.
    ii. Advertisers. LifeOnKey provides segmentation of information for the use of advertisers. This information is anonymous.
  LifeOnKey may disclose only aggregated, anonymous user statistics for prospective partners, advertisers, researchers and other third parties, and all for lawful purposes which do not violate any of your privacy rights
 
4. LifeOnKey's Cookie policy.
  a. Cookies are small files which are placed automatically on your computer by a website. A cookie provides the website with information about the computer on which it is placed.
  b. LifeOnKey uses cookies only for login purpose and security reasons, and do not keep any personal data on our servers. If a user removes the option to use cookies, he would not be able to login to the application.
 
5. LifeOnKey Security Measures
  a. LifeOnKey websites and servers are protected both physically and technologically. The LifeOnKey system is secured with SSL technology, compatible with HIPAA standards. This is designed to ensure the confidentiality of your information.
  b. Secured login: in order to enter your personal file you must enter both your username, ID# and personal password. You may decide to authorise other healthcare providers (your physicians, etc.) to enter your files. In order for them to be able to enter your file you must give permission through our website. Alternatively, you may wish to give them your user username, ID# and password.
  c. Access to your information is only available to you with your username, personal ID# and password, or to those you have authorised to enter your files.
  d. Our website and your information are encrypted with a SSL technology. The Secure Socket Layer (SSL) technology ensures that all communication between your local computer and our server is encrypted - hence others trying to view this information with no authorisation, they will not be able to read it.
  e. Upon leaving our website you may encounter a standard warning displayed by your browser alerting you to the fact that you have left a "secure" website and entered a non secure one.
 
Encryption Validity Certificate - our site has an SSL Web Server Certificate which enables you to verify that the site is properly secured. This Certificate means that Verisign has checked and verified our registration documents and domain name. Always check a site's certificate before entering any sensitive information.
 
6. Changes in this Privacy Policy:

If we decide to change our privacy policy, we will post those changes on our home page and any other appropriate places so you are aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. We reserve the right to change this policy, so please review it frequently.
  b. By using this site, you consent to the collection and use of the anonymous information as set forth in the privacy policy. Of course, by using the LifeOnKey system, you consent to using and sharing your personal and medical data in accordance with these terms.
